Personal information refers to all information collected by WestchesterRx that identifies you as an individual. This includes, but is not limited to, contact information (name, address, e-mail address, telephone number, etc.), date of birth and medical history as it relates to the drug card program.

We collect this data in order to enroll eligible individuals in our discount drug card program. Personal information must be gathered in order for WestchesterRx to provide services; however, you may chose to enroll by fax or mail instead of online.

WestchesterRx will not share your personal information to third parties, other than those parties that partner with WestchesterRx to provide drug card discounts. We may use the information collected through the enrollment process, as well as through plan administration, in aggregate to identify pharmaceutical benefit trends. These trends may be shared with third parties.

WestchesterRx reserves the right to change, modify, add or remove portions of this policy at any time.
